Your First Visit

  • What to expect

    You can count on a warm greeting on your first visit to Midroni Family Dental Care, and on every visit after, too! Our welcoming staff will make sure that you and your loved ones feel confident and secure about the treatment you receive and we are always happy to answer your questions.

    We are conveniently located at 551 Eglinton Avenue West, with metered on-street parking. A Green P parking lot is a three-minute walk away, at Eglinton and Castle Knock Rd.

  • What to bring

    You can save time by completing your Patient Registration Form before your first visit. You may e-mail the form, or bring it along on your initial visit.

    Before your first appointment, we also require you to complete a separate Patient Medical History Questionnaire and include a list of any prescribed medications. The X-ray and Patient Chart Release form allows us to request X-rays and your case history from your previous dentist.

    This form is for patients registering for the first time at our practice. Info includes your name, address, insurance details, etc.
